Outreach & Home Support
Outreach Support is the final destination in the care pathway towards independence. The key focus of our outreach support is to enable greater independence and development of skills. This approach enables us to ensure that the placement is safely sustained and that over time we can reduce support hours and care package costs further.
Outreach support is where the individual lives in a single tenancy in the community with support going in based on individual need. Support can range from a few hours a week to 24/7 daily support.
Each individual is provided with outcome-focused support during the day and/or night in line with their individual assessed need. The purpose of this provision is to be flexible: accommodating changing needs and building around the individual’s activities and social networks.
